Human factors and mechanical failures behind USCG detentions

The U.S. Coast Guard (USCG) publishes a monthly list of IMO-reportable vessel detentions, available on its website. Reports for January through April 2012 show multiple detentions of vessels where issues with OWS were noted. Both human factors and mechanical mix-ups figure in January’s write-up of the Panamanian flagged bulker Clipper Kamoshio (Class NK) at New Orleans. The USCG noted: “The engineering department was not familiar with the operation and maintenance of the 15ppm bilge ....
